Please fill this out knowing that I want to do my best to support you as much as I can. I can’t help what I don’t know is a frustration for you. I’m sure I won’t be able to help with every situation, but hopefully we can work together to make sure planning and teaching these sweet children won’t be a burden for any teacher.

1. What supplies do you use most often from the teacher supply closet (corner of the big room nearest the paint).

2. What supplies would you like to have more of?

3. What materials/manipulatives do you use from the teacher closet (and/or from other teachers’ materials that are at school?)

4. Do you use the plans from the filing cabinet in Susie’s office? If so, which ones would you prefer to keep?

5. Which themes/crafts for the 3’s class, do you wish were modified or changed?

6. Which themes/crafts for the Pre-K class, do you wish were modified or changed?

7. What materials, manipulatives, toys, etc do you wish you had in your classroom?
